  • Care Planning

    • Patients with a chronic medical condition or complex care needs can benefit from a “General Practice Management Plan”.  You are provided with a copy to help you better manage your health by understanding the goals and actions planned by your doctor in consultation with you.   The plan is updated regularly.  It enhances communication between your doctor and other health professionals who are part of your health management team such as specialists, allied health and hospital staff.

    • A “Team Care Arrangement” is another part of care planning for patients with a chronic medical condition or complex care needs.  This enables effective communication between your GP and your other treating health professionals, and gives access to up to 5 medicare subsidised allied health services in a 12 month period.  Please discuss with your doctor if you would like further information regarding these initiatives

  • Nursing Services

Our practice nurses are an integral part of your health care team and strive to provide holistic care for all our patients. They provide amongst other things:

    • Initial assessment when required for acute illnesses.

    • Health education, monitoring and advice for those with ongoing health conditions.

    • Specific nurse-led clinics, including 4 year old health checks, immunisation clinics, and a clinic for our patients with diabetes.

    • Diagnostic and management procedures including wound care, assisting our doctors with surgical procedures, ear syringing, lung testing (spirometry), cardiac testing (ECGs) and collection of blood tests and other pathology when required.

    • A home visit for an annual health assessment is available to those over 75 years of age.  This involves a comprehensive assessment of your overall health and living needs by our nurse Janet, and a follow up appointment with your doctor to discuss the assessment and make arrangements for any needs that have been identified. This service is bulk-billed and enhances your care tremendously.

  • Health Initiatives for specific populations in our community

    • We participate in other health care initiatives such as the “Closing The Gap” program for people who identify as Aboriginal and/or Torres Strait Islander, and the Department of Veterans Affairs Co-ordinating Care program. Please enquire with your doctor or our practice manager if you would like more information on these schemes.
